Here are the Official E-Class Record Templates pursuant to DO 8, s. 2015 - Policy Guidelines on Classroom Assessment for the K to 12 Basic Education Program.

1. Please use CTRL + S command in your keyboard after using the file, in order that the FILENAME and LOCATION OF THE FILE is saved on the same folder.

2. DO NOT RENAME (change) the file name, unless you open the Summary Grading Sheet at the same time, so that the links of each file may not be distorted and the functionality of these links are very important.
